Certificate of the institution of the chapel and entail of Diogo Saraiva and Isabel Mendes, his wife, issued by Baltazar de Sequeira Coelho, tabelião das notas in Lamego, following a petition presented by Domingos Lourenço Lágeo.

Chapel foundation deed by which Diogo Saraiva and his wife Isabel Mendes incorporated several properties and rents of cereal, wine, chestnuts and olive oil, in order to fulfill the chapel's obligations. They wanted to build, at their own expense, the chancel of the hermitage of Nossa Senhora da Conceição de Penelas, on the outskirts of Lamego. The administrators would be obligated to maintain the hermitage and to ensure the cult. They designated their son, Doutor Domingos Saraiva, to be its first administrator and to be responsible for choosing between one of his brothers, or between one of his nephews, the sucessor of this entail.
