Capela de D. Aldonça, mulher de Miguel de Bettencourt e Freitas, instituída em 1634-06-08.
Institui dois vínculos: um com encargo de uma missa anual, imposto na fazenda da Junqueira, administrado por seu marido e depois por seu irmão Pedro Alves Uzel; outro com pensão de duas missas na Semana Santa, imposto nos seus aposentos na Ribeira de Tabua, que deixa ao dito seu irmão.
O testamento refere: a escrava Maria, forra; uma dívida de vinte mil réis procedente da venda de um escravo ao mercador Diogo Freitas Correia.
